Covid Vaccinations

The COVID-19 vaccine is no longer something you can have whenever you like. England has moved to a seasonal model: the NHS opens a vaccination campaign, invites the groups who benefit most, and closes it again. Outside a live campaign the national booking service is shut and pharmacies hold no NHS stock, which is why you may find nothing bookable if you look in the middle of the year.

Every campaign is decided the same way. The Joint Committee on Vaccination and Immunisation weighs how much serious illness the vaccine still prevents in each group and publishes advice, the government accepts or amends it, and NHS England then commissions the campaign. That is why the eligible list has changed almost every year, and why no pharmacy can tell you in advance exactly what the next one will look like.

Our chemist on Pinner Road sits between North Harrow and Harrow town centre and takes part in the NHS COVID and flu vaccination service. People come to us from West Harrow and Rayners Lane as well as the town centre, very often to get both vaccines done in a single visit rather than making two separate trips.

The free NHS offer is much narrower than it was during the pandemic. Recent campaigns have covered adults above a set age threshold, most recently 75 and over, together with residents of care homes for older adults and anyone aged six months and over who is immunosuppressed as defined in the UKHSA Green Book. The threshold has been raised as evidence has shown less benefit in younger, healthier people.

Groups that were once included no longer are, and this is what catches people out at the counter. Adults aged 65 to 74, people with long-term conditions such as asthma, diabetes, COPD or heart disease, and frontline health and social care workers have all been removed. Pregnancy on its own does not make you eligible either, though vaccination is considered safe in pregnancy and while breastfeeding for anyone who qualifies on other grounds.

Because the criteria move, the honest answer to "am I eligible?" is that it depends on the campaign. Ask us, or check nhs.uk once a campaign opens. NHS COVID-19 appointments are managed centrally through the National Booking Service. You can book online at nhs.uk, through the NHS App, or by calling 119 free of charge, where interpreters are available. Pharmacies delivering the NHS service are required to use that booking system, so the slot is arranged there rather than over our counter, and some parts of the country also run walk-in sites.

Spacing between doses matters. A seasonal dose is normally given at least three months after your last COVID-19 vaccine, whichever product that was, and people who are severely immunosuppressed may be advised to have additional doses on specialist advice. You do not need to leave a gap after a COVID-19 infection as long as you have recovered from it.

If a flu vaccine is due as well, have them together. Co-administration is recommended, and any pharmacy offering the NHS COVID service must also offer flu vaccination, so it is one trip and one set of paperwork. The common side effects are mild and short-lived: a sore arm, tiredness, headache, aching muscles and sometimes a mild fever. Most people are back to normal within a week, and reactions tend to be milder in older adults than in younger ones. Serious allergic reactions are very rare and our vaccinators are trained to manage one.

One thing has changed that surprises almost everybody. The routine 15-minute wait after a COVID-19 vaccination was suspended in the NHS programme, and that suspension now covers all the vaccines in use, so most people can leave straight away. Observation is still required if you have a personal history of significant allergy, and everyone is told not to drive for 15 minutes afterwards because fainting can follow any injection.

The vaccines used are reformulated to match the variants in circulation, which is another reason a fresh seasonal dose is offered rather than relying on one you had years ago.
